Certificate of Registered Matters that officially confirms registration of incorporation or any document similar thereto. (Please attach a Japanese translation.) ^*1

Documents to be submitted by the applicant certifying the power and the signature of the representative of the applicant. (Please attach a Japanese translation.) ^*2

Documents to that the applicant must be the entity mentioned under Article 44, Paragraph 1, Item 13 of the Act on Book-Entry Transfer of Corporate Bonds and Shares, etc. (Act No.75 of 2001) ^*3

  The Certificate of Registered Matters must be submitted to JASDEC within 3 months from the date it was issued.

  The Documents must be submitted to JASDEC within 3 months from the date they are issued.

  Please note that an entity applying for the Foreign Indirect Account Management Institution regarding Book-Entry Transfer System for Shares, etc. must meet the following requirement(s).
  The applicant must be the entity mentioned under Article 44, Paragraph 1, Item 13 of the Act on Book-Entry Transfer of Corporate Bonds and Shares, etc. (Act No.75 of 2001) ^*4

  Article 44, Paragraph 1, Item 13 (extraction from the provisional translation of the Act on Book-Entry Transfer of Corporate Bonds and Shares, etc.)
  a person licensed or registered or subject to a similar disposition in a foreign state pursuant to foreign laws and regulations, to manage another person's Bonds or Other Securities or rights similar to Bonds or Other Securities in that foreign state, as designated by the competent minister.

An applicant should appoint a Contact Person, who communicates with JASDEC for the applicant relating to the Book-Entry Transfer System for "Corporate Bonds" who has an address in Japan.

An applicant should complete the Application Form and submit it to JASDEC via the applicant's Contact Person in Japan.
